The most simple and best way to cure a real sore throat in adults is using a gargle. As children can not gargle properly it is better to use a throat compress or homeopathic remedies for them.
If you do not see improvement from home remedies for sore throat overnight, please visit your doctor, especially if you suspect strep throat. A real strep throat may have severe complications like inflammation of the kidneys or heart or joints.
Using gargles should bring improvement immediately, latest in one or two hours.

Maybe one of the most effective sore throat remedy is a hot gargle with Cayenne Pepper.
Most effective and quick acting sore throat treatment is using app. 5 shakes of Cayenne Pepper powder in a large cup of hot water (or herbal tea or saline solution or lemon juice) and gargle every 10-15 minutes with the spicy elixir. You can swallow each time a little bit. Gargling with cayenne pepper may cause a burning sensation of lips and tongue. In order to avoid this unpleasant feeling it is good to keep milk or yogurt in your mouth after gargling until the burning disappears.
If you do not have access to Cayenne pepper use 15-20 drops of Tabasco sauce in a large glass of warm water and the healing effect will be nearly the same.
If you do not have neither Cayenne nor Tabasco, simple chili peppers or hot paprika or white/gray pepper are similarly useful. All the above mentioned pungent spices contain capsaicin, which dulls the pain and has anti-viral and anti-bacterial properties.
Ginger (Zingiber officinalis) tea is also a great choice. Simmer 10-15g peeled and grated fresh ginger root or dried ginger powder in a large cup of water for ten minutes. Strain, cool down and give optionally a teaspoon of fresh lemon juice and/or honey and/or a pinch of Cayenne pepper. Use as a gargle every 20-30 minutes.
CAUTION: As Cayenne pepper is a natural blood thinner, be cautious and do not use Cayenne gargles before surgery or dental treatment.
If you can not stand hot stuff: vinegar or other sour, acidic substances such as lemons are nearly as effective as pungent gargles. Strong acidity (acidic pH) kills germs. That makes them effective sore throat cures.
Hot VINEGAR like: diluted Apple Cider Vinegar or Pickle Juice (brine from a jar of pickles) may bring immediate relief and can soothe within the first few sips. They are truly great sore throat home remedies. Add 1-2 tablespoons of Apple Cider Vinegar to a large cup of hot water and gargle as hot as possible. You can also swallow a tiny bit of it. Alternatively you can heat up the pickle juice and sip it and/or gargle.
Fresh lemons are often used as sore throat remedy. Just suck on a lemon or gargle with freshly pressed lemon juice diluted with equal amount of warm water. You also may eat freshly squeezed lime or lemon juice mixed with equal amount of honey. In my practice I found vinegar a bit more effective than lemon.
CAUTION: vinegar and lemon are strong acidic substances and they can damage the enamel of your teeth. Please, rinse your mouth thoroughly after gargling.
Lemon juice dries up the throat, don't use it, if you have a dry throat irritation.
It may happen that the bugs in your throat are acid-resistant and the infection does not improve from vinegar or lemon. You can use a strong alkaline solution in order to kill viruses and bacteria, since they are rarely alkaline-resistant.
Using Potassium Carbonate or if you don't have it, Sodium Bicarbonate - better known as baking soda - solution will bring amelioration soon. Add 1/2 teaspoon of Potassium Carbonate or Sodium Bicarbonate to a large glass of hot water. Gargle on a regular basis every 20-30 minutes. Alkaline gargle should not been swallowed.
Most widespread sore throat remedy is the old fashioned salt water gargle but I suggest that you should give additional ingredients e.g. cayenne pepper or crushed garlic to the basic saline solution. It has mild antiseptic properties and draws water out of mucus membranes. It can reduce swelling and helps to clear up phlegm.
Caution: salty gargle of high salt concentration dries up your throat, do not use it for dry throat irritation.
Quick first aid is to shake a little table salt directly into your mouth to the back of your throat. Do not swallow, let the salt dissolve there. Salt draws out moisture from the cells and kills bacteria. Be aware that salt dries up the lining of your throat.
If you prefer herbs, you can make a herbal gargle easily. Just prepare a strong herbal tea and add some honey and/or fresh lemon and/or a pinch of cayenne pepper and/or salt and use as a gargle.
Best sore throat remedy is Slippery elm (Ulmus rubra) bark or Marshmallow (Althaea officinalis) root. They contain mucilage that coat the lining of the throat, ease soreness and calm inflammation. To make a tea, mix 2 teaspoons of the dried root or bark in two cups of water, bring to boil and simmer gently for 15 minutes, strain, cool down and use as gargle or drink.
Old home remedy and traditional recipe is the following recommendation: Simmer 1 teaspoon of dried marshmallow root and 1 teaspoon of chopped fresh peeled ginger root in 250 ml water for 10-15 minutes in a covered pot. Remove from the heat, cover and steep for additional 10-15 minutes. Strain, cool down add honey and/or lemon and/or 1/2 teaspoon of salt and/or a pinch of cayenne pepper and gargle every 20-30 minutes if helps.
If you combine different substances in one gargle you can make an even more effective sore throat remedy. Feel free to experiment!

I would like to share with you a well-proved and fast-acting remedy combination for the early stages of strep throat or really painful, severe sore throat.
Take alternately every 2-3 hours
2-2 pills. Let them dissolve under your tongue. Take them for one day. Do not eat 5-10 minutes before and after taking the homeopathic sore throat remedy. Stop taking the next day if it does not help, and visit a homeopath or doctor. If you notice marked improvement take the remedies 3 days more but less frequent.
Increasingly popular Wild Oregano (Origanum vulgare) Essential Oil is a truly effective sore throat treatment. Soak a cotton swab in oil of Oregano and dab it directly onto the inflamed area. Alternatively you can make a gargle and put 2-3 drops of Wild Oregano essential oil in a cup of hot water. Do not forget to stir the water-oil solution well. Keep in mind and do not forget our good old Garlic, which has outstanding antiseptic effects and has the power to cure both viral and bacterial sore throat. Garlic is a great remedy for any kind of infection and is also a fantastic sore throat remedy. At the first signs of a coming sore throat, place one clove of garlic in your mouth between your cheeks and teeth and keep it there for a while. Repeat it several times a day.
If you fancy munching some garlic, you can eat two raw cloves each day without anything or chop finely and add to hot soup just before serving. If you cannot stand the smell of garlic then opt for garlic supplements: take garlic capsules or tincture for 4-5 days.
Another option for a smelly but effective sore throat treatment is: Crush one clove of raw garlic and add 2 teaspoons of honey and freshly squeezed lemon juice. Take from the mixture 1 teaspoon several times a day.
A lovely and nearly forgotten old folk remedy is Baked Lemon with Olive Oil.
Take an organic lemon and poke some deep holes with a fork into it. Place it in an ovenproof dish and put it in the oven on low heat (150-170C) and bake it until juice bubbles out of the holes. Take it out from the oven, cool down and squeeze the juice out. Put it in a jar and add equal amount of organic extra virgin olive oil and some manuka honey. Shake well.
